Firstly, we would like to thank you for your interest in collaborating on the 6th LuCE Report “LIVING WITH LUNG CANCER: EUROPEAN EXPERIENCES”. We are pleased to have your support in this initiative, and hope that it will be a positive experience for you. The LuCE Report is an annual initiative led by Lung Cancer Europe (LuCE), with the purpose of raising awareness regarding the main challenges faced by people impacted by this disease across Europe. This 6th edition will explore the experiences of people living with lung cancer: patients and caregivers (family, friend, relative, couple, etc.). This initiative is led by Lung Cancer Europe (LuCE), which has commissioned Fundación MÁS QUE IDEAS (a non-profit organisation a non-profit organisation which is a member of LuCE) to conduct the interviews. Therefore, Fundación MÁS QUE IDEAS is the data processor, as per Regulation 2016/679 of the European Parliament and of the Council, of 27 April 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data. The interview we propose to you will be recorded for a more suitable analysis of your input. Therefore, we will ask for your consent after reading the information below. The purpose of this agreement is to safeguard your data protection rights and to allow Fundación MÁS QUE IDEAS to record and use the interview pursuant to the following terms and conditions and after having obtained your consent. CONFIDENTIALITY AGREEMENT AND AUTHORISATION. PERSONAL DATA PROCESSING Project information: Fundación MÁS QUE IDEAS is coordinating the 6th LuCE Report, whose purpose is to achieve a better understanding of the experiences of people living with lung cancer. Data is being collected through: • a self-filled online survey through the “Surveymonkey®” platform; • and 10 interviews with people with lung cancer and 10 interviews with caregivers. This agreement refers and is exclusively limited to your participation in the interview. Interview characteristics: - The purpose of the interview is getting to know the interviewee’s perceptions, experiences and insights in relation to their life experiences after being diagnosed with lung cancer, while identifying areas for improvement in the health care system. - Participation in the interview is confidential and anonymity of the interviewee is guaranteed. Likewise, the findings will not allow for identification of the interviewees. - Interviews will be conducted over the phone or by telematic means, at the interviewee’s choice. If the interviewee does not speak English, the interview may be carried out through a questionnaire written in the interviewee’s language. - The interviewee will be free to answer any questions he/she wishes, and to leave the interview at any time. - The interview will take place on the date and at the time agreed with the interviewee in May, June or July 2021. - The interview will take approximately 30-40 minutes. - The interview will be recorded for correct transcription of the information. Fundación MÁS QUE IDEAS represents and warrants that, after fulfilling the purpose for which the interviewee provided his/her data, said data will be destroyed pursuant to the applicable laws. - Participation is charitable, there being no valuable consideration in return. The information gathered during the interview will help to better understand lung cancer challenges and will be included in a publicly accessible report (presentation: November 2021), always guaranteeing the interviewee’s anonymity. Your rights as interviewee: In compliance with Regulation 2016/679 of the European Parliament and of the Council, of 27 April 2016, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, the personal data collected during the interview will be processed and incorporated to a database owned by Fundación Privada MÁS QUE IDEAS, with the sole purpose of using the information provided solely and exclusively for the 6th LuCE Report. Data will not be transferred, sold or otherwise disclosed to any third party, except for the providers of Fundación Privada MÁS QUE IDEAS providing services to the latter (such as hosting companies, etc.), which may not process said data for their own purposes under any circumstance whatsoever. The interviewee will be entitled to exercise, at any time, his/her rights of access, rectification, erasure, objection, portability and restriction of processing by sending a written request by post (to the address indicated at the bottom of this agreement) or e-mail (lasideasde@fundacionmasqueideas.org), together with a copy of their identity document, and, if applicable, to file a complaint with the relevant Regulatory Authority. Furthermore, please be advised that the aforesaid providers may deliver services outside the EU. Therefore, it is likely that your data will be processed outside the European Union. Fundación MÁS QUE IDEAS agrees to keep secret and absolutely confidential any Confidential Information provided by the interviewee or other information accessible and disclosed by the latter, and agrees to refrain from transferring or disclosing said Confidential Information, in whole or in part, to any individual or legal entity, without having previously anonymised said data in such a way as to prevent said data from being associated with you or identified.
